Role Overview Gotham Enterprises Ltd is seeking a dedicated and experienced Licensed Clinical Social Worker (LCSW) to join our compassionate team remotely from Beaumont, Texas. Key Responsibilities

• Conduct comprehensive assessments of patients and families to identify emotional, social, and environmental factors affecting their health.
• Develop and implement individualized treatment plans that address the unique needs of patients in hospice care.
• Provide counseling and support to patients and families, helping them navigate the complexities of end-of-life care.
• Collaborate with a multidisciplinary team to enhance patient care and improve overall quality of life.
• Maintain accurate and timely documentation of patient interactions and treatment progress.
• Participate in staff meetings and contribute to the continuous improvement of hospice services.

Required Qualifications

• Master’s degree in Social Work from an accredited institution.
• Current Licensed Clinical Social Worker (LCSW) certification.
• Proven experience in hospice or palliative care settings preferred.
• Strong communication and interpersonal skills, with the ability to build trusting relationships with patients and families.
• Proficiency in using electronic health record (EHR) systems.
