The Role We are seeking a compassionate and skilled Licensed Clinical Social Worker/Medical Social Worker to join our team. This position involves providing comprehensive psychosocial assessments, counseling services, and care coordination for patients and their families within a virtual medical setting. The ideal candidate will have experience in healthcare environments and a strong commitment to patient-centered care. Experience in Cardiology preferred. An Arizona license and availability to work standard business hours (9 AM - 5 PM) in Pacific Time are required.
Responsibilities
Clinical Services Conduct comprehensive psychosocial assessments to identify patient and family needs Develop and implement treatment plans in collaboration with interdisciplinary teams Conduct discharge planning and coordinate community resources Care Coordination Collaborate with physicians, nurses, and other healthcare professionals to ensure holistic patient care Coordinate referrals to community resources, support services, and specialty care Facilitate communication between patients, families, and medical teams Participate in interdisciplinary team meetings and case conferences Patient and Family Support Provide emotional support and counseling to patients facing medical diagnoses Educate patients and families about medical conditions, treatment options, and coping strategies Assist with advance directive planning and end-of-life care discussions Documentation and Compliance Maintain accurate and timely clinical documentation in proprietary technology and/or electronic health records Ensure compliance with HIPAA, state regulations, and hospital policies Complete required assessments within established timeframes
Qualifications
Education and Licensure Master's degree in Social Work (MSW) from a CSWE-accredited program Current Arizona state licensure as a Licensed Clinical Social Worker (LCSW) or equivalent Valid state professional license in good standing Experience Minimum two years of clinical social work experience preferred Experience in healthcare/medical settings strongly preferred Knowledge of medical terminology and healthcare systems Experience with electronic health record systems
