Clinical Documentation Specialist - Medical Scribe

Key Responsibilities
As a Medical Scribe, your primary responsibilities will include:
- Documenting Patient Encounters (80%): Join the provider in the exam room to observe patient visits, meticulously documenting encounters in real time. Craft structured notes that encompass the history of present illness, assessments, plans, and physical examinations. Accurately assign appropriate CPT and ICD-10 codes, and prepare After Visit Summaries to ensure patients understand their care pathways.
- Clinical Documentation Improvement (10%): Request and review medical records. Utilize CVS Health's population health management tools to enhance the quality of clinical documentation. Participate in Daily Huddles and Clinical Documentation Reviews, consulting with providers to identify areas of improvement.
- Administrative Support (10%): Assist your provider and care team with administrative tasks, including placing orders and managing referrals, thereby ensuring a seamless care experience for patients. Support the care team in various clinical documentation-related responsibilities as assigned.
Patient Population / Client Focus You will work closely with diverse patient groups, emphasizing value-based care, particularly for chronic conditions. Your role is integral in building a comprehensive understanding of community health needs, which enhances not only individual patient outcomes but also fosters better clinical practices within the team.
Qualifications
Applicants must possess a Bachelor's Degree. A solid understanding of medical terminology and common medications gained through a pre-medical degree, or previous clinical experience is required. The role demands strong communication skills, coupled with advanced listening capabilities and a familiarity with electronic health records systems. Preferred Experience While prior clinical experience, such as shadowing and volunteering, is strongly preferred, previous scribe or transcription experience is beneficial but not mandatory. Fluency in languages such as Spanish, Polish, or Russian will be advantageous in serving our diverse patient population.
Compensation and Benefits
The hourly salary for this role ranges from USD 15 to 20, commensurate with experience.
