ACO Triage Nurse (remote)
ACO Triage Nurse Millennium Physician Group Full Time-Panama Shift 11am to 11pm, includes every other weekend Remote Work Essential Duties and Responsibilities
- Participate in the After-Hours On-Call Team rotation
- Answer all incoming calls immediately and give correct advice
- Provide Post-Acute Care Outreach
- Assist Hospitalist's Patient Outreach
- Facilitate Prescription Refills
- Assist RN Care Management Teams with patient assessments and/or monthly follow-up calls, when needed
- Monitor Patients enrolled in the Patient Monitoring Program
Required Knowledge and Experience
- Registered Nurse license, multi-state license required
- License must always be kept current
- 5+ Years of clinical experience with 1+ year(s) of case management experience preferred
- ER/ICU/Triage experience is preferred.
- RN ED experience preferred
