- Pay Range $20.00 – $22.25 per hour What You’ll Do As a Weekend Virtual Care Nurse (LPN), you will play a key role in reducing hospitalizations and improving patient outcomes through proactive remote support and clinical intervention.
Responsibilities
Include
- Conduct virtual patient visits and clinical assessments
- Perform triage and identify early warning signs or risk trends
- Support chronic disease management and patient education
- Monitor telehealth data and escalate concerns appropriately
- Help prevent avoidable ER visits and rehospitalizations
- Collaborate with interdisciplinary care teams
- Document patient interactions accurately and timely
- Participate in care planning and performance improvement initiatives
- Support development of virtual care best practices Qualifications Required
- Active, unencumbered Florida LPN license
- Minimum of 2 years of home health experience
- Experience with triage and hospitalization prevention
- Strong clinical judgment and communication skills
- Comfortable using telehealth platforms and EMR systems
- Reliable internet connection and private remote workspace Preferred
- Experience working with high-risk or medically complex patients
- Knowledge of chronic disease management
