Work Environment

This role is fully remote, allowing you to manage your schedule while servicing clients where they feel most comfortable. While our facility is centered in a hospice setting, the flexibility of remote work enables you to provide care tailored to individual client needs without the constraints of a traditional office environment. You may occasionally conduct visits to clients for assessments and consultations, ensuring continuous care is maintained.

Key Responsibilities

  • Conduct comprehensive assessments to evaluate the psychosocial needs of elderly clients, focusing on emotional well-being, social integration, and mental health status.
  • Develop and facilitate personalized care plans alongside clients and their families, ensuring that specific goals and preferences are met.
  • Provide emotional support and counseling to seniors, assisting them in navigating transitions, grief, and challenges related to end-of-life care.
  • Advocate for clients’ rights, ensuring access to essential community resources and additional support services to meet their needs.
  • Collaborate effectively with interdisciplinary teams, including nurses and healthcare professionals, to coordinate service delivery, enhancing overall care.
  • Monitor client progress through regular check-ins via video calls or in-person visits, assessing living conditions and addressing safety concerns.
  • Facilitate family communication, resolving any conflicts that may arise regarding the care of senior clients.
  • Educate clients and their family members about available community resources and support programs aimed at enhancing engagement and quality of life.
  • Maintain detailed and accurate documentation of client interactions, assessments, and care plans, ensuring compliance with ethical and regulatory standards.
  • Stay current with ongoing training in social work practices and hospice care regulations, ensuring the highest standard of service delivery.

Patient Population / Client Focus We primarily focus on serving seniors in hospice care, addressing the unique emotional and psychological challenges associated with aging, chronic illness, and end-of-life issues. Our approach emphasizes compassion, dignity, and respect, ensuring clients are equipped with tools to manage anxiety, depression, and other mental health concerns while navigating this critical stage of life.

Qualifications

  • A Master’s degree in Social Work from an accredited institution.
  • A current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • A minimum of two years of experience in social work, preferably in a community or geriatric healthcare setting.
  • In-depth understanding of social work methodologies, aging-related issues, and available community resources.
  • Strong interpersonal and communication skills, with an ability to build rapport and trust with clients and families.

Preferred Experience

  • Experience working directly with seniors or within hospice care environments.
  • Familiarity with electronic health record systems for efficient client documentation.
  • Knowledge of local resources and support services for elderly clients.

Compensation and Benefits

The salary for this full-time position ranges from USD 70,000 to 100,000 annually, commensurate with experience.
