[Remote] District Supervisor, Innovations Waiver - Medicaid and State Southern Region

Trillium Health Resources is a Managed Care Organization serving individuals with serious mental health needs and disabilities across North Carolina. They are seeking a District Supervisor for the Innovations Waiver to oversee a team providing support for members with Intellectual or Developmental Disabilities and Traumatic Brain Injury, ensuring they receive whole-person care.
Responsibilities
- Provide direct supervision and oversight for assigned team of localized, multi-disciplinary team supporting members with intellectual/developmental/TBI disabilities
- Ensure staff utilize workflows as required in Trillium Connections to complete Care Management comprehensive assessment and care plans for members as care needs change
- Establish a team based, person-centered approach to coordination of care to be implemented by all team members that effectively manages a member's physical health, behavioral health, and social determinant of health needs using established workflows and processes effectively and efficiently and in accordance with NCQA procedures
- Monitor teams to help ensure integration of care through the establishment of a multi-disciplinary team (e.g. member, caretaker/legal guardian, PCP, behavioral health and IDD providers, specialists, nutritionists, pharmacy, etc.) to meet the member's needs
Skills
- Experience in a managed care environment
- Knowledge of Intellectual or Developmental Disabilities or Traumatic Brain Injury
- Ability to provide direct supervision and oversight for a multi-disciplinary team
- Experience in implementing industry standard, evidence-based and best practices
- Familiarity with care management comprehensive assessment and care plans
- Ability to establish a team-based, person-centered approach to coordination of care
- Knowledge of NCQA procedures
- Experience in monitoring teams for integration of care
