Registered Nurse (RN)- Per Diem
About the position Vitality Hospice & Palliative Care is a clinician-owned and operated hospice agency seeking a compassionate and experienced Per Diem Registered Nurse (RN) to join our interdisciplinary team. In this role, you will provide skilled hospice nursing care to patients in their homes while collaborating closely with physicians and care team members to reduce symptom burden and enhance comfort, dignity, and quality of life for patients and their families. If end-of-life care is meaningful to you, we would love to welcome you to Vitality. This is a Per Diem position and a field-based role with local travel. Mileage reimbursement is provided.
Responsibilities
- Provide professional nursing care, including admissions, comprehensive assessments, and ongoing patient evaluations
- Develop, implement, and revise individualized plans of care
- Collaborate with the hospice physician and full interdisciplinary team to ensure appropriate interventions
- Oversee and support care provided by home health aides, social workers, and chaplains
- Educate patients and caregivers to promote safety, comfort, and understanding
- Maintain accurate, timely documentation and ensure regulatory compliance
- Participate in quality improvement initiatives to enhance clinical excellence
Requirements
- Active Pennsylvania Registered Nurse (RN) license (required)
- Minimum 1 year of RN experience
- Strong communication, assessment, and interpersonal skills
- Ability to work independently in the field
- Valid driver’s license, reliable transportation, and current auto insurance
- Willingness to travel within assigned service areas
Nice-to-haves
- Hospice or home health experience preferred (not required)
