Schedule: PRN Shift: Day (United States of America) Address: 7480 W COLONIAL DR City: ORLANDO State: Florida Postal Code: 32818 Job Description: Schedule: Per Diem (6 shifts in a 6 week period) Shift: Days, 7:00 am - 7:00 pm Primary Job Responsibilities: Conduct comprehensive clinical assessments to evaluate patients' physical, mental, and emotional health. Develop and implement individualized care plans based on assessment findings and patient needs. Coordinate multidisciplinary care by facilitating collaboration among healthcare professionals, including physicians, nurses, social workers, and therapists. Monitor patient progress, regularly review, and adjust care plans based on patient responses and outcomes. Communicate effectively with patients, families, and the healthcare team to ensure everyone is informed and involved in the care process. Document patient information accurately, including assessments, care plans, interventions, and outcomes in the patient's medical record. Administer medications and treatments safely, ensuring adherence to protocols and patient safety.
