PRN Nurse

This position could work during the day throughout the week or cover on-call nights and weekends occasionally.
Responsibilities
- Provide comprehensive care coordination and support to hospice patients and their families.
- Assess patient needs, develop individualized care plans, coordinate care services, and provide emotional support throughout the end-of-life journey.

Requirements
- RN licensure for the state of Indiana
- Clean driving record, and valid driver’s license and auto insurance
