Role Description This contract Physical Therapist, Occupational Therapist role focuses on providing high-quality home care and rehabilitation services to patients across Southwest Florida. Day-to-day responsibilities include conducting comprehensive patient assessments, developing individualized treatment plans, and implementing evidence-based physical therapy interventions to restore function and mobility. The PT and OT will monitor progress, adjust care plans as needed, collaborate with physical therapist assistants and C OT Assistants and document clinical findings and outcomes in accordance with agency and regulatory standards. The PTA and COTA will provide skilled therapy interventions under the direction of the supervising PT/OT, following the established plan of care, monitoring patient response to treatment, communicating changes in patient status, and documenting each visit in accordance with agency policies and regulatory standards. The role also involves educating patients and caregivers on home exercise programs, safety strategies, and ongoing self-management, while collaborating closely with home health agencies and interdisciplinary care teams to ensure coordinated, consistent care.
Qualifications
- Strong foundation in Physical Therapy/Occupational Therapy to provide safe, effective, evidence-based treatment.
- Demonstrated experience in Rehabilitation and Patient Care, with a focus on functional recovery and quality of life.
- Knowledge and practical skills in Home Care, including delivering therapy in home-based and remote settings.
- Current, unrestricted Physical Therapist/Occupational Therapist license in the state of Florida, and compliance with all regulatory requirements.
- Experience in home health or community-based care is highly beneficial.
- Excellent communication, documentation, and time-management skills, with the ability to work independently as a contract clinician.
- Comfortable with using digital tools for scheduling, documentation, and remote coordination with agencies and care teams.
