Work Environment

This full-time position offers a remote work setting, allowing for flexibility while you engage with patients and collaborate with medical teams from the comfort of your home. The role requires availability for a variety of shifts, including day, evening, and weekend rotations, on an as-needed basis. You may also have on-call responsibilities, ensuring that patient care needs are met around the clock.

Key Responsibilities

  • Conduct comprehensive psychosocial assessments to identify patients' emotional, social, and financial challenges.
  • Provide individual and group counseling to patients, focusing on adjustment issues, grief, and coping strategies.
  • Collaborate closely with interdisciplinary medical teams to deliver holistic patient care that optimizes treatment outcomes.
  • Coordinate with community organizations to arrange post-discharge services, facilitating a smooth transition for patients and ensuring continuity of care.
  • Offer educational resources and emotional support to patients and their families regarding available treatment options and community resources.
  • Participate actively in hospital committees to provide insight and expertise on matters related to social work.
  • Implement discharge plans that connect patients with appropriate community resources tailored to their needs.
  • Maintain accurate and timely documentation in accordance with hospital policies and legal requirements.

Patient Population / Client Focus Your work will involve supporting a diverse patient population within the school healthcare system, a community known for its unique blend of educational and mental health challenges. You will primarily focus on students facing emotional, developmental, and family-related issues that impact their well-being and academic success. Building rapport with patients and their families is essential to ensure high-quality care and effective interventions.

Qualifications

  • A Master’s degree in Social Work from an accredited institution.
  • Current licensure as a Licensed Independent Social Worker (LISW) in the state of Ohio.
  • Strong knowledge of medical terminology, mental health disorders, and crisis intervention strategies.
  • Excellent communication and interpersonal skills, with the ability to build trusting relationships with patients and families.

Preferred Experience

  • Two or more years of experience in a healthcare or hospital setting is preferred.
  • Familiarity with the challenges faced in a school healthcare environment is a plus.

Compensation and Benefits
