• *Job Summary** As an Experienced Licensed Social Worker, you will work remotely from home, partnering with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs. Responsibilities include researching available options and resources for members with financial, transportation, safety, and housing needs. Population spans the entire lifecycle. All member contact is done telephonically or via electronic exchange. This position works to empower members to make positive lifestyle and medical choices and works as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health.
- *Key Responsibilities:**
• Partner with registered nurse case managers to utilize a collaborative process of assessment, planning, and advocacy to meet a member's benefit plan and/or health needs
- Research available options and resources for members with financial, transportation, safety, and housing needs
- Empower members to make positive lifestyle and medical choices
- Work as a team with other clinical staff to assist members with community resources for medical, financial, and behavioral health
- Conduct telephonic or electronic exchanges with members to assess their needs and provide support
- Develop and implement plans to meet members' needs, including connecting them with community resources and services
- Collaborate with other healthcare professionals to ensure comprehensive care and support for members
- *Essential Qualifications:**
• Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) in state of residence
- 2+ years experience in medical social work or case management
- Strong communication and interpersonal skills, with the ability to work effectively with members, healthcare professionals, and other stakeholders
- Ability to work independently and as part of a team, with a strong focus on collaboration and partnership
- Strong assessment and problem-solving skills, with the ability to analyze complex situations and develop effective solutions
- Ability to work in a fast-paced environment, with a strong focus on meeting deadlines and delivering high-quality results
- *Preferred Qualifications:**
• Experience working with diverse populations, including children, adults, and older adults
- Knowledge of community resources and services, including healthcare, social services, and financial assistance programs
- Experience working in a healthcare setting, including hospitals, clinics, and community health organizations
- Strong computer skills, including proficiency in electronic medical records and other healthcare software systems
- Ability to work in a remote environment, with a strong focus on self-motivation and productivity
